Age Limit: Candidates age limit should be 40 years or less than as on 04-01-2016 for Post No. 01, between 20 – 40 years as on 01-12-2015 for Post No. 02, 03, less than 62 years for Post No. 05 & 06 as on 01-01-2016. Relaxation of age will be applicable to SC/ ST/ OBC candidates as per Govt rules.

Educational Qualification: Candidates should possess Graduation from any recognised University for Post No. 01, Higher Secondary & equivalent for Post No. 02 & 03, MBBS for Post No. 04.

How to Apply: Eligible candidates may send their application in prescribed format with attested copies of educational/ training qualification certificates, experience certificate, caste certificate for SC/ ST/ OBC candidates, residency certificate, birth certificate etc & an envelope should be super scribed with “Post Applied for” in bold letters to Office of the Chief Medical Officer of Health & Member Secretary, DHFWS, Alipurduar on or before 19-01-2016 by 04:00 P.M.
